What Are IHSS Services in California?
In-Home Supportive Services is California’s largest home care program, helping seniors and people with disabilities remain safely at home. Understanding IHSS helps eligible Californians access these valuable services.
What IHSS Provides
IHSS provides assistance with daily activities that recipients cannot safely do themselves due to age, blindness, or disability. Services enable people to remain at home instead of moving to nursing facilities.
Personal care services include bathing, grooming, dressing, bowel and bladder care, and other personal needs. These intimate care tasks help with activities many people cannot manage independently.
Domestic services include housecleaning, laundry, shopping, and meal preparation. Maintaining safe, clean homes with proper nutrition supports independent living.
Paramedical services include assistance with medications, injections, and other health-related tasks when ordered by physicians. These services help with medical needs beyond routine personal care.
Protective supervision provides monitoring for those with cognitive impairments who cannot safely be left alone. This service benefits those with dementia or mental health conditions affecting safety.
Who Qualifies for IHSS
California residents receiving Medi-Cal may qualify for IHSS. You must live at home or be able to live at home with IHSS services. You must need assistance with activities that you cannot safely perform yourself.
Eligibility is determined through assessment by county social workers. Assessors evaluate what tasks you need help with and how much assistance is required. Authorized hours are based on assessed needs.
Income and assets affect Medi-Cal eligibility, which is required for IHSS. Those with higher income may qualify for Medi-Cal with share of cost requirements.
How IHSS Works
Recipients receive authorized hours per month for specific services. You can hire your own care provider, including family members in many cases. IHSS pays the provider directly.
You act as the employer, hiring, training, and supervising your provider. This consumer-directed model gives you control over your care. County agencies handle payroll and administrative tasks.
Hours are reassessed periodically and when circumstances change. If your needs increase or decrease, authorized hours can be adjusted.
Applying for IHSS
Contact your county social services agency to apply. In Los Angeles County, call the DPSS IHSS office or apply online. An assessment will be scheduled to determine eligibility and authorized hours.
The application process takes time. Apply as soon as you think you may qualify. Services cannot begin until eligibility is determined and a provider is hired.
