As a PACE organization, On Lok PACE coordinates and delivers all medical and supportive services — primary care, specialists, dental, vision, hearing, prescriptions, therapy, transportation, meals, adult day activities, and in-home care — through a dedicated interdisciplinary care team.

On Lok PACE provides all of the following at no cost for dual-eligible participants:
- Primary medical care and specialist referrals
- All prescription medications
- Dental, vision, and hearing care
- Physical, occupational, and speech therapy
- Mental health and counseling
- Adult day center activities and socialization
- Nutritious meals — at center and home-delivered
- Round-trip transportation
- In-home caregiving and personal care
- 24/7 emergency care coordination

Eligibility
Age 55+, live in the service area, certified as needing nursing-home level care, able to live safely in the community. See full eligibility requirements.
