What Should Seniors Know About Home Health Care Coverage?
Home health care helps seniors receive medical care at home, but understanding what insurance covers can be confusing. Knowing coverage rules helps seniors access appropriate services and avoid unexpected costs.
Medicare Home Health Coverage
Medicare Part A and Part B cover home health services when specific criteria are met. Understanding these criteria helps determine eligibility and set appropriate expectations.
Homebound status is required. Medicare defines homebound as needing help to leave home, leaving home requiring considerable effort, or leaving home being medically inadvisable. You can leave home for medical appointments, religious services, and occasional brief outings while still being homebound.
Skilled care need is required. You must need intermittent skilled nursing care, physical therapy, speech therapy, or continued occupational therapy. Need for personal care alone does not qualify. Skilled care must be medically necessary.
Physician certification is required. A doctor must certify that you need home health care and establish a plan of care. The plan specifies what services are needed and how often.
Medicare-certified agency must provide care. Services from non-certified agencies or individuals are not covered. Ensure your home health agency is Medicare-certified.
What Medicare Covers
Skilled nursing care includes wound care, medication management, injections, and patient education. Nurses assess your condition and coordinate with physicians.
Physical therapy addresses mobility, strength, balance, and function. Occupational therapy addresses daily activities and home safety. Speech therapy addresses communication and swallowing.
Home health aides provide personal care when skilled services are also needed. Aides help with bathing, dressing, and other personal care. Aide services alone do not qualify for Medicare coverage.
Medical social services help with social and emotional concerns affecting health. Medical supplies related to your care are covered.
What Medicare Does Not Cover
Twenty-four-hour care is not covered. Medicare covers intermittent, part-time services, not continuous care.
Personal care only is not covered. Without need for skilled services, personal care does not qualify for Medicare home health.
Homemaker services like cleaning, cooking, and laundry are not covered by Medicare. These are not medical services.
Meals delivered to your home are not covered as home health services.
Costs
Medicare covers home health services at 100 percent with no deductible or copayment when criteria are met. This makes home health an unusually generous Medicare benefit.
Durable medical equipment prescribed as part of home health care is subject to Part B cost-sharing, typically 20 percent after the deductible.
Other Coverage Sources
Medicaid covers home health and additional home care services for eligible low-income seniors. Coverage varies by state.
Private insurance and Medicare Advantage plans may cover home health with varying rules. Check your specific plan benefits.
