2. Functional Abilities Assessment
- The social worker observes the senior performing daily tasks (e.g., walking, dressing, bathing, eating) and asks about mobility, cognition, and support needs.
3. Living Environment Review
- They check the safety and accessibility of the home, identifying hazards or barriers to independence.
4. Health and Medical Review
- Medical history, recent hospitalizations, and any chronic conditions are discussed to determine care requirements.
5. Care Plan Recommendation
- Based on observations and records, the social worker recommends the type and number of IHSS hours and services to be approved.
