What is IHSS protective supervision?

24/7 Care Through IHSS: Understanding Protective Supervision

Protective Supervision is IHSS’s highest level of care, providing round-the-clock monitoring for individuals with severe cognitive impairments who cannot be safely left alone.

What Is Protective Supervision?

  • 24-hour monitoring and intervention to prevent injury
  • For people with mental impairment or illness
  • Requires constant vigilance, not just being present
  • Can receive up to 283 hours monthly (maximum IHSS hours)

Qualifying Conditions

  • Dementia/Alzheimer’s: With wandering or dangerous behaviors
  • Developmental Disabilities: Autism, intellectual disabilities
  • Mental Illness: Severe conditions affecting judgment
  • Brain Injuries: Causing cognitive impairment

Specific Behaviors That Qualify

  • Wandering or getting lost
  • Starting fires or flooding
  • Ingesting harmful substances
  • Self-injurious behaviors
  • Unable to respond to emergencies
  • Poor judgment creating danger

Documentation Required

  • Doctor’s statement (SOC 321 form) confirming:
  • – Specific mental impairment diagnosis
  • – Description of dangerous behaviors
  • – Need for 24-hour supervision
  • Detailed examples of incidents requiring intervention
  • Frequency of dangerous behaviors

Assessment Criteria

  • Memory: Score of 1 (severely impaired)
  • Orientation: Score of 1 (severely impaired)
  • Judgment: Score of 1 (severely impaired)
  • Must show nonself-directing behavior

Important Restrictions

  • Cannot be in school/work/day program full-time
  • Provider must be constantly available
