Role Overview:
We are seeking a skilled Registered Nurse (Home Health RN) with a passion for delivering personalized, in-home healthcare. As part of our team, you will develop and implement patient care plans, provide hands-on clinical support, and ensure the comfort and safety of our senior clients. This role is ideal for a proactive nurse dedicated to uplifting the lives of our patients.
Key Responsibilities:
- Conduct comprehensive clinical assessments, monitor vital signs, and document patient progress.
- Administer medications, treatments, and wound care following established guidelines.
- Collaborate with physicians, caregivers, and family members to coordinate patient care plans.
- Provide patient education on managing chronic conditions and maintaining overall health.
- Ensure compliance with safety protocols, infection control measures, and regulatory standards.
Qualifications:
- Valid RN license in the state of California.
- Experience in home health, geriatric care, or related field is preferred.
- Excellent communication, clinical judgment, and problem-solving skills.
- Proficient with electronic health records (EHR) and care documentation.
