Role Overview:
we are seeking a Home Health Registered Dietitian to provide personalized nutrition plans and guidance for seniors at home. Your expertise in meal planning and dietary management will help our clients improve their health, manage chronic conditions, and enjoy nutritious, balanced meals that fit their lifestyle.
Key Responsibilities:
- Assess seniors’ nutritional needs, dietary restrictions, and health goals.
- Create individualized meal plans and provide guidance on healthy eating habits.
- Collaborate with caregivers and families to ensure dietary compliance and understanding.
- Offer education on managing conditions like diabetes, hypertension, and heart disease.
- Monitor patient progress and adjust recommendations as needed.
Qualifications:
- Registered Dietitian credential and California license.
- Experience working with seniors or home health patients preferred.
- Strong communication, education, and coaching skills.
