Role Overview:
As a Physician Assistant (Geriatric Home Care), you will visit patients in their residences to perform health assessments, assist with care plans, and provide ongoing medical support under physician supervision. Your clinical expertise and compassionate approach will help ensure seniors receive timely, appropriate care that meets their unique needs.
Key Responsibilities:
- Conduct in-home patient assessments, gather medical histories, and evaluate symptoms.
- Implement treatment plans, manage medications, and provide preventive care advice.
- Work closely with supervising physicians and interdisciplinary teams to coordinate services.
- Educate patients and families on chronic disease management and lifestyle modifications.
- Maintain accurate records and ensure compliance with healthcare regulations.
Qualifications:
- Active Physician Assistant license in California.
- Experience in geriatrics, primary care, or home health preferred.
- Strong clinical judgment, communication, and patient education skills.
- Proficiency with EHR documentation and collaborative care models.
