Role Overview:
As a Home Health Physical Therapist (PT), you will design and implement individualized therapy programs that help seniors improve strength, balance, and mobility. Your expertise in creating targeted exercises and rehabilitation plans will enable patients to maintain their independence and enjoy a more active, fulfilling life.
Key Responsibilities:
- Conduct thorough patient assessments to identify physical therapy needs.
- Develop customized treatment plans focused on mobility, pain management, and fall prevention.
- Provide hands-on therapeutic exercises, stretching, and balance activities in-home.
- Educate patients and caregivers on at-home exercises and safe mobility techniques.
- Track patient progress and adjust treatment plans to achieve optimal outcomes.
Qualifications:
- Valid Physical Therapist license in California.
- Experience working with older adults or in a home health setting preferred.
- Excellent communication, patience, and motivational skills.
- Ability to create and adapt therapy plans based on patient progress.
