Role Overview:
As a Nutrition Educator (Home-Based), you will meet with seniors to evaluate their nutritional needs, create individualized meal plans, and offer guidance on healthy eating habits. You will collaborate with caregivers and other healthcare professionals to support overall well-being.
Key Responsibilities:
- Conduct nutritional assessments and identify dietary goals.
- Develop meal plans tailored to health conditions, preferences, and cultural factors.
- Educate patients and families on proper food choices, portion control, and meal preparation.
- Collaborate with dietitians, nurses, and home aides to ensure consistency of care.
Qualifications:
- Degree or certification in nutrition, dietetics, or related field.
- Experience working with elderly populations or in home health environments.
- Excellent communication and patient education skills.
