Role Overview:
As a Nurse Practitioner (Home Health NP), you will conduct in-home assessments, diagnose common conditions, and develop individualized treatment plans for seniors. You will collaborate with interdisciplinary teams, oversee medication management, and provide patient and caregiver education, ensuring optimal patient outcomes and enhanced quality of life.
Key Responsibilities:
- Perform comprehensive in-home health assessments and medical evaluations.
- Prescribe medications, order diagnostic tests, and initiate treatment plans.
- Collaborate with physicians, nurses, therapists, and social workers to coordinate care.
- Educate patients and families on disease management and preventive measures.
- Monitor patient progress, adjust care plans, and maintain thorough documentation.
Qualifications:
- Valid Nurse Practitioner license and certification in California.
- Experience in geriatrics, home health, or primary care strongly preferred.
- Excellent clinical judgment, communication, and patient advocacy skills.
- Proficiency with EHR systems and telehealth platforms.
