Role Overview:
As Memory Care Coordinator, you will design and coordinate our memory care program for seniors with Alzheimer’s, dementia, and mild cognitive impairment. You will work with families, clinical teams, and community partners to deliver coordinated dementia care across in-home and community settings.
Key Responsibilities:
- Coordinate care plans for seniors with dementia and their family caregivers
- Conduct cognitive screenings and refer for diagnostic workups when appropriate
- Lead caregiver support groups and dementia-friendly community programming
- Connect families with respite care, adult day programs, and memory care resources
- Train staff and volunteers on dementia communication, behavior strategies, and safety
- Partner with the Alzheimer’s Association, USC ADRC, and other dementia organizations
Qualifications:
- Bachelor’s in gerontology, social work, nursing, or related field
- 2+ years working with dementia patients and families
- Certified Dementia Practitioner (CDP) or willingness to obtain within 6 months
- Strong understanding of dementia stages, behavior, and care strategies
- Group facilitation skills
