Role Overview:

We seek a compassionate, skilled, Licensed Vocational Nurse (LVN) to join our team. The LVN will provide essential nursing care to our senior clients, working closely with physicians, registered nurses, and other healthcare professionals. This role is ideal for a dedicated individual with strong clinical skills and a passion for improving the health and well-being of seniors.

Key Responsibilities:

  • Provide direct patient care, including administering medications, monitoring vital signs, and performing basic nursing procedures.
  • Assist in the development and implementation of individualized care plans for senior patients.
  • Collaborate with physicians, RNs, and other healthcare providers to ensure comprehensive patient care.
  • Educate patients and their families about health conditions, medications, and care plans.
  • Maintain accurate and confidential patient records in compliance with healthcare regulations.
  • Assist with routine medical procedures, such as wound care, catheterization, and injections.
  • Monitor patients for changes in condition and report any concerns to the healthcare team.
  • Support the emotional and physical well-being of patients through compassionate and patient-centered care.
  • Ensure compliance with all healthcare policies, procedures, and safety standards.
  • Participate in ongoing professional development to stay current with best practices in nursing care for seniors.

Qualifications:

  • Valid Licensed Vocational Nurse (LVN) license in the state of California.
  • Proven experience as an LVN, preferably in a senior or long-term care setting.
  • Strong clinical skills, including medication administration, wound care, and patient monitoring.
  • Excellent communication and interpersonal skills, with a focus on compassionate care.
  • Ability to work collaboratively with a multidisciplinary healthcare team.
  • Knowledge of healthcare regulations, documentation, and compliance requirements.
  • Proficiency in electronic health record (EHR) systems.
