Role Overview:
As Hospice Bereavement Coordinator, you will design and lead our bereavement program supporting families during the 13 months after a loved one’s passing. You will provide one-on-one counseling, facilitate support groups, and connect families with grief resources.
Key Responsibilities:
- Provide individual bereavement support to families for up to 13 months following a loss
- Facilitate ongoing grief support groups (in-person and virtual) for different demographics
- Conduct bereavement assessments to identify complicated grief and refer to clinical care
- Develop and distribute grief education resources (mailings, calls, anniversary cards)
- Coordinate annual memorial events for families served by our hospice program
- Document bereavement interactions and maintain compliance with Medicare hospice regulations
Qualifications:
- Master’s degree in social work, counseling, psychology, or related field
- Licensure (LCSW, LMFT, LPCC) preferred; certification in thanatology (CT) a plus
- 2+ years experience in hospice, palliative care, or grief counseling
- Strong active-listening skills and ability to sit with intense emotion
- Group facilitation experience
