Role Overview:
As a Home Health Pharmacist, you will review medication regimens, identify potential interactions, and provide consultation to patients, families, and healthcare teams. Your guidance ensures seniors adhere to their prescriptions and experience better health outcomes.
Key Responsibilities:
- Evaluate medication profiles, dosages, and potential drug interactions.
- Offer medication counseling, adherence strategies, and patient education.
- Collaborate with physicians, nurses, and other providers to optimize medication therapy.
- Maintain accurate documentation and comply with pharmacy regulations.
- Assist in the development of policies for safe medication management.
Qualifications:
- Licensed Pharmacist in California.
- Experience with geriatric patients or home-based pharmacy services preferred.
- Strong communication, analytical, and problem-solving skills.
- Knowledge of medication management tools and EHR systems.
