Role Overview:
We are seeking an experienced Geriatric Care Manager to oversee and coordinate comprehensive care plans for elderly clients. You will work closely with families, healthcare providers, and community organizations to ensure seniors receive the best possible support while maintaining their independence at home.
Key Responsibilities:
- Develop and manage individualized care plans, addressing healthcare, social, and emotional needs.
- Coordinate with physicians, nurses, social workers, and other providers to streamline services.
- Assist families in navigating community resources, insurance, and financial guidance.
- Monitor client progress and adjust care plans as needed for optimal outcomes.
- Advocate for seniors’ well-being, ensuring patient-centered, compassionate care.
Qualifications:
- Bachelor’s degree in Social Work, Nursing, Gerontology, or related field preferred.
- Experience in care management, home health, or eldercare coordination.
- Strong communication, organizational, and problem-solving abilities.
- Knowledge of local community resources and healthcare systems.
