Director of Nursing (DON) – Lead Home Health Clinical Operations

Role Overview:
As Director of Nursing, you will provide clinical leadership for our entire home health program — supervising nursing staff, ensuring CMS compliance, leading QAPI, and shaping clinical strategy. You report directly to the CEO and partner with the Medical Director on clinical quality.
Key Responsibilities:
- Provide clinical supervision and leadership to RNs, LVNs, HHAs, and therapy staff (40+ clinicians)
- Ensure compliance with CMS Conditions of Participation, CA Title 22, and Joint Commission standards
- Lead the QAPI program: chart audits, performance improvement, Star Ratings, HHCAHPS
- Develop and update clinical policies, procedures, and competencies
- Manage clinical hiring, onboarding, performance evaluation, and progressive discipline
- Represent the agency in state surveys, accreditation reviews, and payer audits
Qualifications:
- Current CA RN license + BSN required; MSN strongly preferred
- 5+ years home health nursing including 2+ years in management
- Familiarity with OASIS, CMS regulations, Joint Commission, and CA DPH home health rules
- Strong leadership, communication, and conflict-resolution skills
- COS-C, CHPN, or CPHQ certification a plus
