Every service is delivered at no cost, regardless of insurance status.
Role Overview:
As Diabetes Educator, you will teach seniors and families how to manage Type 1, Type 2, and pre-diabetes through nutrition, glucose monitoring, medication management, and lifestyle changes. You will work in seniors’ homes and via telehealth across LA County.
Key Responsibilities:
- Conduct comprehensive diabetes self-management education (DSME) for newly diagnosed and existing patients
- Teach blood glucose monitoring, insulin administration, and continuous glucose monitor (CGM) use
- Develop personalized nutrition plans accounting for cultural preferences and food access
- Coordinate with primary care, endocrinology, and home health teams on care plans
- Lead group classes (in-person and virtual) on diabetes prevention and management
- Document patient progress, A1C trends, and education milestones in our EHR
Qualifications:
- Current CDCES (Certified Diabetes Care and Education Specialist) credential
- Underlying RN, RD, or pharmacist license
- 2+ years diabetes education or clinical experience with diabetic patients
- Strong patient-education skills; comfort with low-literacy and non-English speakers
- Cultural humility around food, body image, and family dynamics
