Role Overview:
As Cardiac Care Coordinator, you will lead our home-based cardiac care program for seniors with heart failure, atrial fibrillation, post-MI recovery, and other cardiovascular conditions. You will work with cardiologists, primary care, and home health teams to keep seniors stable at home.
Key Responsibilities:
- Conduct telephonic and in-home cardiac assessments including BNP, weight, edema, vitals tracking
- Educate seniors and families on heart failure self-management, medication adherence, and dietary sodium
- Coordinate care between cardiologists, primary care, and home health teams
- Monitor patients using remote cardiac devices (BP cuffs, weight scales, implantable monitors)
- Identify and respond to early decompensation signs to prevent hospital readmissions
- Document encounters in EHR and contribute to cardiac quality metrics
Qualifications:
- Current CA RN license + BSN preferred
- 2+ years cardiac nursing (telemetry, cardiac stepdown, CHF clinic, or home cardiac care)
- AACN Cardiac Medicine certification (CMC) preferred
- Strong patient-education skills around medications and lifestyle changes
- ACLS certification
