What Should Seniors Know About Prostate Health?

Prostate problems become increasingly common as men age, causing urinary symptoms that affect quality of life. Understanding prostate conditions helps men recognize symptoms, seek appropriate evaluation, and make informed decisions about treatment options.

Understanding the Prostate

The prostate is a walnut-sized gland located below the bladder and surrounding the urethra in men. It produces fluid that nourishes and transports sperm. The prostate’s location means that changes in the gland directly affect urination.

The prostate grows throughout life, with significant enlargement common after age 50. This natural growth can compress the urethra and affect bladder function. By age 80, about 90 percent of men have some prostate enlargement.

Benign Prostatic Hyperplasia

Benign prostatic hyperplasia, called BPH, is non-cancerous prostate enlargement causing urinary symptoms. Symptoms include frequent urination especially at night, weak urine stream, difficulty starting urination, feeling of incomplete bladder emptying, urgency, and dribbling after urination.

BPH symptoms range from mild to severe. Mild symptoms may need only monitoring. Moderate to severe symptoms often benefit from treatment. Untreated severe BPH can cause bladder damage, urinary tract infections, and kidney problems.

BPH Treatment Options

Lifestyle modifications help mild symptoms. Limiting fluids before bed, reducing caffeine and alcohol, and double voiding may provide relief. Avoiding decongestants and antihistamines that worsen symptoms helps.

Medications effectively treat many men with BPH. Alpha-blockers relax prostate and bladder neck muscles, improving urine flow. 5-alpha reductase inhibitors shrink the prostate over time. Combination therapy may be more effective than single medications.

Minimally invasive procedures and surgery are options when medications fail or symptoms are severe. Various procedures reduce prostate tissue blocking urine flow. Treatment choice depends on prostate size, symptoms, and patient preferences.

Prostate Cancer

Prostate cancer is common in older men, with most cases growing slowly and not causing problems during the patient’s lifetime. However, some prostate cancers are aggressive and life-threatening. Distinguishing slow-growing from aggressive cancers guides treatment decisions.

Prostate cancer often causes no symptoms in early stages. Advanced cancer may cause urinary symptoms similar to BPH, blood in urine or semen, erectile dysfunction, or bone pain if cancer has spread.

PSA blood tests and digital rectal exams can detect prostate cancer, but screening decisions are complex. Screening finds some cancers that would never cause problems, leading to unnecessary treatment with side effects. However, screening also finds aggressive cancers early when they are curable.

Screening decisions should be individualized based on age, health status, life expectancy, and personal preferences. Men should discuss screening pros and cons with healthcare providers. Screening is generally not recommended for men with less than 10-year life expectancy.

When to See a Doctor

Seek evaluation for urinary symptoms affecting quality of life, blood in urine or semen, difficulty urinating, or inability to urinate, which is a medical emergency. Do not assume symptoms are just aging. Treatable conditions often cause these symptoms.
