What Hospice Services Are Covered by Medicare for Seniors?

Medicare provides comprehensive hospice coverage for beneficiaries with terminal illnesses, offering comfort-focused care at little to no cost. Understanding what Medicare’s hospice benefit includes helps families make informed decisions and ensures dying patients receive all available support during their final months of life.

Eligibility for Medicare Hospice Benefits

To qualify for Medicare hospice coverage, a beneficiary must be enrolled in Medicare Part A and have a terminal illness with a life expectancy of six months or less if the disease follows its normal course. Two physicians, including the hospice medical director, must certify the terminal prognosis. The patient must sign a statement choosing hospice care and agreeing to forgo curative treatments for the terminal condition.

Choosing hospice does not mean giving up all medical care. Patients can still receive treatment for conditions unrelated to their terminal diagnosis. They can also revoke hospice at any time if they decide to pursue curative treatment, and can later re-enroll in hospice if appropriate.

Services Covered Under Medicare Hospice

Medicare hospice benefits are comprehensive and designed to address all aspects of end-of-life care. Physician services include visits from the hospice medical director and attending physician for symptom management and care planning. Skilled nursing care provides regular visits from hospice nurses who monitor symptoms, administer medications, and educate families about what to expect.

Medical equipment and supplies related to the terminal illness are covered, including hospital beds, wheelchairs, oxygen equipment, and wound care supplies. Medications for pain control and symptom management are provided by the hospice, usually with minimal copayments. Physical therapy, occupational therapy, and speech therapy are available when needed for comfort and function.

Home health aide services provide personal care assistance with bathing, dressing, and grooming. Medical social services help families cope with emotional and practical challenges. Counseling services, including dietary counseling, are available for patients and families. Spiritual care from hospice chaplains supports patients of all faiths or no faith.

Respite Care and Inpatient Services

Medicare hospice includes respite care, which provides short-term inpatient care to give family caregivers a break. Respite care can be provided in a Medicare-approved facility for up to five consecutive days at a time. This benefit recognizes the exhaustion caregivers often experience and helps prevent burnout.

When symptoms cannot be managed at home, Medicare covers inpatient hospice care in a hospital or hospice facility for acute symptom management. Once symptoms are controlled, the patient returns to home-based hospice care.

What Hospice Does Not Cover

Medicare hospice does not cover treatments intended to cure the terminal illness. Emergency room visits and hospitalizations related to the terminal condition are not covered unless arranged by the hospice. Care from providers not arranged by the hospice team is generally not covered.

However, Medicare continues to cover treatments for conditions unrelated to the terminal diagnosis. If a hospice patient breaks a hip, for example, Medicare would cover treatment for that injury.

Cost to Patients

Medicare hospice benefits have minimal cost-sharing requirements. There is no deductible for hospice services. Patients may pay small copayments for prescription drugs, typically no more than five dollars per medication. Respite care may require a five percent copayment. Most hospice services have no copayment at all.
