Role Overview:
As a Telehealth Nurse, you will conduct virtual assessments, provide patient education, assist with medication management, and coordinate care among healthcare professionals. Your role enhances accessibility and continuity of care for seniors, improving their overall health outcomes.
Key Responsibilities:
- Perform remote patient assessments, triage, and symptom management.
- Provide education on chronic disease management, wellness, and preventive care.
- Coordinate referrals, follow-ups, and care plans with on-site providers.
- Maintain accurate telehealth documentation and comply with privacy regulations.
Qualifications:
- Registered Nurse license in California.
- Experience in telehealth or home health nursing.
- Excellent communication, technology literacy, and assessment skills.
