Role Overview:
As a Palliative Care Nurse, you will provide holistic care and symptom management to seniors with chronic or advanced conditions. By collaborating with an interdisciplinary team, you will ensure patient comfort, dignity, and support at home, focusing on quality of life rather than cure.
Key Responsibilities:
- Assess patient symptoms, pain levels, and overall comfort.
- Develop and adjust care plans to address physical, emotional, and spiritual needs.
- Educate families and patients about palliative care options and coping strategies.
- Coordinate with physicians, social workers, and other providers for comprehensive support.
- Maintain clear documentation and follow regulatory guidelines.
Qualifications:
- Registered Nurse (RN) license in California.
- Experience in palliative, hospice, or chronic care management.
- Strong communication, empathy, and active listening skills.
