Role Overview:
As a Home Health Social Worker, you will help seniors access essential services, provide emotional support, and guide families through healthcare, financial, and social resource navigation. Your involvement ensures a holistic approach to elderly care, improving quality of life and peace of mind.
Key Responsibilities:
- Conduct psychosocial assessments to identify clients’ needs and challenges.
- Connect seniors and families with community resources, support groups, and financial assistance.
- Offer emotional support, counseling, and advocacy to help clients cope with life changes.
- Collaborate with healthcare professionals, caregivers, and external organizations.
- Maintain thorough documentation of client interactions and progress.
Qualifications:
- Master’s in Social Work (MSW) and valid California license (LCSW preferred).
- Experience with geriatric populations or home health services.
- Strong communication, empathy, and resourcefulness.
