Role Overview:
As a Community Health Nurse, you will work within local neighborhoods, offering health screenings, immunizations, and educational programs for seniors. Your outreach ensures that older adults receive guidance on preventive measures, chronic disease management, and access to supportive services.
Key Responsibilities:
- Perform basic health assessments, including vital sign checks and screenings.
- Provide immunizations, coordinate wellness clinics, and promote preventive measures.
- Educate seniors on nutrition, exercise, medication adherence, and chronic disease management.
- Identify community resources and connect patients with in-home care or rehabilitation services.
- Collaborate with local organizations, healthcare teams, and public health agencies.
Qualifications:
- Registered Nurse license in California.
- Experience in community health, public health nursing, or geriatrics.
- Strong communication, health education, and outreach skills.
- Ability to travel locally and engage with diverse senior populations.
